Mississippi High School Activities Association The place for high school sports in Mississippi The MHSAA is providing schools with a Scholar Athlete certificate template, which will allow schools to recognize each member of a Scholar Athlete Team.
Download this Microsoft Word document (Updated for 2018-19) for the individual certificates and this Scholar Athlete Team Award Application Form for the team members with GPA to be submitted to the MHSAA.
The form has a field that will allow you to input a student’s name, then print on a color or black-and-white printer on the paper of your choice.
