Coaches: If your Eligibility Roster Form is not completed and submitted to the MHSAA on time, you will be fined $50.00 immediately. The Eligibility Roster is due in our offive by the end of your season, for every sport that you participate in, and before the first round of playoffs.

This form may be used for all sports.
